The National Health Plan Collaborative Toolkit

"The National Health Plan Collaborative has created this toolkit of resources, lessons, best practices and case studies to help other health plans join the effort to reduce disparities. The toolkit shares what the Collaborative's members have done to develop and test new methods of measuring and addressing racial and ethnic disparities so that other health care decision-makers and leaders can learn from this work, implement these best practices and make the case for addressing the unacceptable differences in health care and health outcomes for health plan members throughout the country."

Culture Care Connection - Bloomington, MN

Increasing the cultural competence of health care providers serving Minnesota's diverse populations http://culturecareconnection.org

Whether you are implementing Culturally and Linguistically Appropriate Services (CLAS) Standards in your organization, looking to enhance culturally sensitive services in general, or wanting to understand a specific patient population better, find actionable tools and resources through Culture Care Connection—Minnesota’s New Online Learning and Resource Center, http://culturecareconnection.org.

Culture Care Connection is the first Minnesota-focused online resource and learning center aimed at improving culturally competent care. Developed by Stratis Health and funded by UCare, Culture Care Connection supports health care providers, staff, and administrators in their ongoing efforts to provide culturally competent care in Minnesota. The site:

  • Brings together Minnesota-specific resources and connections to Minnesota organizations that support health care facilities in developing and refining their cultural competence programs.
  • Provides a broad range of cultural resources and actionable tools that support health care providers, staff, and administrators in their ongoing efforts to provide culturally competent care in Minnesota.
  • Provides a free, online assessment for health care organizations to determine how well they meet the Office of Minority Health’s national standards on Culturally and Linguistically Appropriate Services (CLAS). The tool provides immediate results with actionable recommendations.
  • Shares information on Minnesota’s unique blend of cultures, building an understanding of Minnesota’s changing demographics. Its fact sheets help health care providers learn more about the background, religious and cultural beliefs, and common health issues of the underserved populations in Minnesota, including African American, American Indian, Hispanic/Latino, Hmong, Russian, and Somali. A DVD training series is available that offers insights into the health beliefs and norms of Hmong, Hispanic/Latino, and Somali in Minnesota.

  • Offers profiles of each Minnesota county so health care providers can learn more about the characteristics of the communities they serve. Demographic characteristics and socioeconomic status can influence a person's health behaviors, affect access to health care, and indicate an increased likelihood of certain health conditions. Each profile describes quantitative data, outlines what providers need to know, and makes suggestions for developing culturally sensitive services and processes necessary to support delivery of equitable and effective health care.

We invite you to use Culture Care Connection and enhance culturally competent care in Minnesota!
